The Yellow Bracelet care scheme was developed by a team of clever folks, who wanted to make sure that people like me and you who have a Care Package provided by a local authority or private company, do not experience any unnecessary overnight stays in hospital, when instead we should be looked after by our chosen care provider, at home, or in our residence. It's a service developed to Empower Health & Personal Well-Being. It makes me feel safe, in the understanding that in the event of an emergency any of the Health & Social Care professionals who I authorise to view and access my details, can do so with ease, by using a standard smartphone camera to scan a secure code - no confusing apps or passwords needed. In the last 6 months when I've been using the Yellow Bracelet service, it's helped me and my family by: Reducing avoidable admission to hospital after a fall which I had Reducing the common delays discharging me from Hospital, as I already have a care package in place Reducing the number of bed days spent in hospital Putting my kids' minds at rest, knowing that In Case of Emergency (ICoE) the Health & Social care professionals can access my care package details and Medication tracking data in real-time Giving me a new lease of life

With the patience and support of the…

With the patience and support of the Sundown Solution staff, the web-site was easy to navigate and form was easy to complete. It's a very good 'tool' with all the information stored safely in one place but accessible in times of medical emergencies.

The Yellow Bracelet is brilliant

The Yellow Bracelet is brilliant! It was really easy to sign up and i was able to fill in all of my mum's care package information on her behalf. This just really gives me peace of mind for if anything happens to her.

Accessing people's vital information…

Accessing people's vital information can be challenging at the best of times; important records in regards to health conditions are paramount for people to receive the right care at the right time. Having the yellow bracelet allows this information to be accessed by, paramedics and the hospital team. These records provide up to date medical history which allows for clear evidence and the aim for reducing unnecessary hospital admissions.
